It may be possible with relation widget and some layout… but not that easy for that specific final render I think.

I dug really deep into the web and all configurations and cannot find a way to do this. Tagging m-kuhn as an expert who might have a clue whether this is possible.

The best you can get at the moment is childrens through relations Attribute form - QField Ecosystem Documentation
Adding a table would require a new relation editor widget implementation (along the lines of ordered relation editor)
